Why the MLW World Middleweight Title Is Vacant

The MLW World Middleweight Championship was declared vacant on April 4, 2025, after the reigning champion Mistico was ruled medically unfit to compete following sustaining injuries.

Under Major League Wrestling’s rules of the sport, champions must be able to defend their crown for all contractually executed championship bouts. With doctors projecting the former titleholder’s recovery to extend well beyond a reasonable timeframe, Mistico relinquished the belt.

The decision—never taken lightly—opens the door for a new era. MLW President Cesar Duran and the championship committee – made up of promoters from around the world – are discussing plans for the road to crown a new champion.
